Consent Form - Brisbane

We’re over the moon that you like one of our artist’s work! Before the fun can begin, we’ll need to make sure we have a few things on record.

By completing this form, you agree that….

1 - You are at least 18 years of age.

2 - You are of sound mind and willing to allow your artist to perform their work.

3 - You are willingly allowing the artist to make a permanent change to the appearance of your skin.

4 - You are not pregnant.

5 - You are not breastfeeding.

6 - You have read and agreed to your artist’s own terms and conditions and understand that they do not supersede these terms and conditions.

7 - You have eaten a filling meal in the last 4 hours.

8 - You have not had alcohol in the last 24 hours.

9 - You have not had drugs in the last 24 hours.

10 - You have not had blood thinners in the last 24 hours.

11 - You have not undergone chemotherapy in the last 12 months.

12 - You do not have a medical condition that would negatively impact the artist’s work.

13 - You do not have an illness that would risk the health of your artist or any other people at Artists Guild.

14 - You will surrender your body to your artist so they may perform their work and understand that there may be complications such as muscle pain, heavy bleeding, and other reactions during and after the session.

15 - You will follow the directions your artist provides to you during the session.

16 - You will remain quiet and not cause a disturbance during the session.

17 - You will leave the studio peacefully if requested by a member of the Artists Guild for any reason and will need to contact your artist for options after leaving the premises.

18 - You understand that pricing is subject to change if your artist deems the workload to be different compared to what you booked.

19 - You understand that the work performed on your body may not look like a reference due to factors such as your skin quality, your reaction to ink, your movements during the session, and your care routine after the session.

20 - Your artist is not performing their work as a medical professional and is not capable of providing professional medical advice relating to the work that will be performed on your body. However, they will provide care advice in a medium of the artist’s choice that is to be treated as suggestions.

21 - Neither the Artists Guild nor your artist are liable for any complications that may arise during the healing process relating to the work that will be performed on your body. You are solely responsible for following the care advice provided by your artist and are encouraged to contact your artist via their preferred medium if you require care support, but are highly encouraged to seek medical attention if complications arise.

22 - Neither the Artists Guild nor your artist are liable for any claims, damages, or actions relating to the work that will be performed on your body.

23 - Neither the Artists Guild nor your artist are liable for the accuracy of formal spelling, traditional meaning, historical authenticity, or any other matter of accuracy relating to the work that will be performed on your body.

24 - The Artists Guild is not liable for any claims, damages, or actions. You are dealing with an independent business which is operated by the artist you confirmed in this form. The artist is a contractor that is solely responsible for themselves and the goods and services they provide.

25 - These terms and conditions are governed by the laws of Queensland, Commonwealth of Australia to the maximum extent permissible by law. All disputes will be resolved exclusively through fair and binding arbitration administered by an arbitration organization selected by the Artists Guild. Clients waive the right to participate in any class action, representative proceeding, or trial by jury. If the jurisdiction the work was performed in does not allow arbitration, or waivers of class actions, representative proceedings, or jury trials, those parts in these terms and conditions will not apply in that jurisdiction. However, all other provisions in these terms and conditions will still apply.
